Description

The Registered Nurse coordinates all aspects of the client’s home care and interacts with other disciplines as needed.  He/she provides professional care to the client, as indicated by Agency policies, State and Federal laws/ regulations.  In-home, admission visits and eligibility determinations are part of the RN’ duties, as is the development of the Care Plan. The RN also prepares paperwork necessary for Agency policies and federal/state laws and assists with the training and supervision of Care Aides.

Qualification / Prerequisites:

Home care may include light housecleaning, laundry, meal preparation, transportation, companionship, respite and advice on such things as nutrition, cleanliness and household activities.

Home maker:  housecleaning, laundry, meal preparation, transportation, companionship and respite,

Live-In Home Care Aides provide “around-the-clock” care to clients, who prefer to receive needed services in their own homes instead of having to go into an Assisted Living Facility.

Responsibilities / Activities:

Assist with the activities of daily living and personal care including:

Bathing

 Ambulation

Mouth care                   

Dressing                          

Exercise

Hair care                       

Feeding

Toileting

Nail care                       

 Positioning                       

 Medication reminding

Skin care                      

  Transferring                     

Vital signs and Blood Pressure
